What exactly makes this proc?  Successful Dodge doesn't seem to do it (like it does with Stinging Backfist.)

Also why isn't Thousand Fists showing up in my Reaction window?  (I hate using the window but the hotkeys are pretty hard to see in 1680.)

Backfist is off dodge,  Quivering Palm is off Parry
relog is your thousand fists isn't showing up
