If the mob incoming (say an add) agros on you before you strike then it won't land. But in general if you are not pulling and are waiting on incomming mobs, you can nerve strike.

If someone pulls 2 things with a bow, you can always nerve strike one of them.

Forget about Pulling as a Monk and handling adds. Forget about trying to solo a group of 2 or more mobs using Nerve Strike. Granted we aren't a stealth class, but there could be some combination of a Drunken move to riddle the mob followed by a Nerve Strike IN COMBAT to stun and run, or take one out of the fight. But every pull in Graystone, the tank would pull with his bow, and I would Nerve Strike the other one in the group on its way back to our group.

I can believe that it might mess up and add you to agro as there is lots of bugs with defensive target. Such as a bard with group invis changes his defensive target it will uninvis the person that was. Maybe if your defensive target is the one thats pulling he is adding you to mob agro.
I do not keep the tank (usually the puller) as my defensive target simply because as a Drunken Monk the only thing I can give him is Goading slap (+800 Hate) that has too long a cool down timer to be useful, so I keep the Healer there where I can spam my neg hate attack if he gets agro. I will check the next time I am grouped if this does indeed put me in combat. That being said, I use nerve strike quite a bit on incoming mobs with no failures, so definatly you are not put in combat simply because you are grouped with someone in combat, if that was the case then it would never work in a group situation, after un-FD'ing your members are still in combat then so would you. When someone else in my group pulls I only enter combat when I perform an action. Unless of course the mob hits me with an aoe or for some reason comes after me instead of the puller. Other then that I am out of combat untill I do something.

You always could, the ability requires you to not be in combat, it dosen't require the mob to be out of combat.
In-Combat and Out of Combat are various states that dictates quite a lot about what you're capable of; it's like a healer throwing of the revelation they could use their OOC ressurection or summon while the tank was out pulling despite the tank being agroed.
